Bridges

Bridges is a program providing long term community residential placement to children, youth and adults with intellectual disabilities. The individuals served may also have medical, mobility, mental health or behavioral issues. We match the needs of the individual with the skills of the caregivers, with the intention of creating long term placements. Placements may be with individual caregivers, couples or families as appropriate.

Activity groups

We offer weekly music therapy during the Fall, Winter, Spring, some Summer programming and a weekly Social Group.

Program Objective

To promote and facilitate community-based living situations for individuals who require extra support.

Service approach

  • Our service approach is to provide supports to assist individuals in achieving and maintaining independence, safety, and stability in the community. We develop person-centered plans and resources in consultation with the individual, referring agencies, family members, and school or day program personnel. Bridges offers the advantage of working both with adults and with children in care. When eligible individuals are approaching the age of majority, we are able to offer continuity of service, maintaining the new adult in his/her caregiver home.
  • A Case Manager is assigned to every participant to coordinate services and to offer overall support to the individual and his/her caregiver.
  • Bridges encourages ongoing connection with families.

Referrals

Individuals must be referred by a Child and Family Services agency or one of the regional offices of Manitoba Family Services, Community Mental Health Services, School Divisions or Children's Special Services.

Referral packages should be sent to the attention of the Program Manager, Bridges. Please include recent social, medical, family and school/vocational information, plus a covering letter outlining the request for service. Individuals must be eligible for appropriate levels of funding.
